I have cholangiocarcinoma. And primary sclerosing cholangitis (PSC). The tumors are still small (2 cm), but are in both the left and right hepatic ducts and down through the distal common bile duct. I've met with Mayo for a transplant evaluation, Columbia/Pres for a second opinion, and Memorial Sloan Kettering regarding non-surgical chemo options. I would need a liver transplant and whipple, en bloc. I should hear back from Mayo on whether they will accept me for a possible transplant later this week.
Mayo has already said my tumors are 'outside of their protocol' because they are visible below the cystic duct, and they said they are 'leaning toward no'. Columbia is willing to do the surgery if I don't eventually fall out of their program (due to spread, etc.), but I've heard Mayo is just so much better given they have more experience with cholangiocarcinoma. Mayo has also said that because of where the cancer is, the chance of recurrence is high, so the chances of 5-yr survival are far below 50% (they didn't specify more than that). Per the surgeon at Mayo, they might say 'no' because of all the stress of doing the transplant + whipple, plus the high risk of complications and recurrence, could actually make my quality of life far worse, which is a really important consideration. But even if Mayo says no, Columbia might still do the transplant / whipple, if I want to go that route.
Here is my dilemma... I'm trying to decide between taking the chance on a combined transplant / whipple knowing that complications could put me in the hospital (about 1,200 miles away from my wife/children) for a large percentage of the time I have left, or whether I should just go with Sloan Kettering near my home for systemic chemo treatment that might buy me a little time but wouldn't cure anything (I am ineligible for the more targeted chemo/radiation at MSK because it would worsen biliary strictures given my PSC). Clearly, there is some small chance a transplant/whipple would be curative, but I would hate to miss my last year or two with my family by being stuck in the hospital with complications (the odds of success are low). MSK would allow me to be with my family, but I would only have a year or two of life at most (per MSK). My wife and I cry every day thinking about this impossible choice. I know you can't make the decision for me, but I would love to hear your thoughts. Thank you.

Every rational, modern person has an idea of themselves. They have an idealized version of themselves in their heads, which they have created over their lifetime. They have done this by attaching themselves to a specific, effective stimulus they have experienced in the physical world. They may have even sought out experiences to develop their character, which means they had a vision of who they wanted to be; they were conscious about it. But, for the most part, this process is an unconscious and sometimes ignorant form of self-actualization. Ignorant as they weren't aware they were doing it, yet they still managed to construct a character. Over time, as they develop their experience and character more and more, they develop an idea of themselves, whether their assumptions are accurate or not. They think, "This is me," as they point to the attachments that define them.
Hopefully, we construct a character that we are proud of. And, the goal is for everyone to be proud of who we have become. It makes perfect sense for everyone to be happy with their accomplishments, success, and relationships with friends and family.
Being proud of who we are should be the goal.
However, for some people, this is not the case.
Some people have not had the best start, have not been born into the best environments, have not had the best education, have not made the best choices, have avoided hard decisions, and for whatever reason, have not created a life that they are proud of.
There are an infinite amount of determinants that amalgamate into inequality in opportunity at the individual level.
That's the way the world is. Some haven't had the same start as others, being born into less fortunate families and countries. Some got ruined along the way, victims to their vices. Some will never be able to return from their darkness, carrying their scars forever. The world isn't fair. It is unfortunate, very unfortunate. But that's how things are, and we can't save everyone. It's a struggle to save ourselves. Some don't even want to be saved.
Such is life: there is suffering on mass and individual levels.
There are parts of our suffering that we have no control over. But, there a parts that we can control. We make decisions every day that either aligns us with our goals or don't. We have the power to control how we respond to a situation, either logically or emotionally. Almost everything can be simplified down to a choice: yes or no. As modern individuals, we are mostly smart enough to know right from wrong. We all know that we should do good in school, eat healthy food, exercise, save money, etc. There are universal 'good' decisions that modern society has created over centuries that still hold true today. Deep down, we all know what to do.
Therefore, the message is...
Do what you know you should.
We all know that we operate, either little or a lot, below our ability and potential. We are all fallible to laziness, fear, ignorance, and inappetence. To add to that list of self-destructive conditions, we fall into our vices, desires, and addictions. The darkness of life is relentless and attacks us from every angle at any time. However, these are conquerable, and despite the struggle, we all know that we can be that little bit, or large bit, better.
For most modern, thinking individuals, the professionals, the suburban dwellers, the money chasers, we are aware of our shortcomings and understand that we can be better. We all have the propensity for education, whether formal or self-education. We all know that we have missed the mark in the past, through the fault of our own indecision. We know that we have achieved less than our potential, and we were dissatisfied with that. Being a modern person, we are well informed about the world and have had all the experiences that would lead to our understanding level.
We are smart. We are able. We are aware. We have never lived in a better time to be proactive and productive.
We all know what we should do.
We all know that we need to make changes to live a more fulfilled life.
We all know that we have the ability to improve our circumstances.
The problem is that we have allowed ourselves to become normalized. We fear leaving our comfort zone. We fear others' opinions. We're too lazy inside the nice, luxurious box we've bought. We don't need to grow, in any way, to continue to survive. And that is precisely our problem.
Deep inside everyone, there is a voice that entices you to climb to greater heights. It knows what you should, and want, to do. Don't continue drowning it out with distractions. It wants to push you in the direction of your dreams, ambitions, and aspirations. We all have it. In some, it will be deeper and harder to find than others. It is not foreign to us; it is a part of our psyche. It is our unconscious, repressed drives to experience this wonderful life in all it's flavors.
Listen to it.
Act with its guidance.
Create a life that you are proud of.
Do as you know you should.

Just got my first Ledger. I am not used to passphrase being attached to my pin. I liked that some other wallets require me to type out my passphrase when doing transactions, connected, through metamask.

Including social and major news outlets. Humans are manipulated by irrational thoughts, especially fear (reptilian brain vs. evolved thinking). Almost any piece of information can be spun to induce fear, and humans cope with fear through defense mechanisms; that literally keeps us sane. However, once defense mechanisms are triggered, the evolved brain gets mostly ignored.
There’s so much money to be made in manipulating this human characteristic, and until we hold media outlets on both sides accountable by their pocketbooks, we are slaves to their thoughts. Don’t call someone else a sheep when you’re falling for this. Ignore the media.
This is unpopular because free press and free media are ingrained in most Western cultures (the comparison being state-controlled media).
